Your insurer pays us directly. Any uncovered remainder is charged to the credit card on file.
Note: Direct billing requires your benefits provider to allow “assignment of benefits.” A credit card is kept on file for any balance not received from your insurer.

Patient responsibilities
- Bring your insurance card or benefits information to your appointment
- Provide a credit card on file to cover any portion not paid by your insurer
- Notify us of any changes to your insurance plan or coverage
- Understand that you are ultimately responsible for any balance not covered by your plan
