What is laughing gas and why is it called that?

Laughing gas is the common name for nitrous oxide, a colourless, odourless gas that has been used safely in dentistry for well over a century. It is called laughing gas because of the mild euphoric, relaxed, and sometimes giddy sensation many patients experience when it takes effect. Not everyone laughs, but most people feel noticeably calmer, warmer, and more at ease within minutes of breathing it in.

Clinically, nitrous oxide is a mild sedative and analgesic, meaning it reduces anxiety and also reduces the perception of discomfort during dental procedures. It does not put you to sleep. You remain fully conscious, can hear and respond to your dentist, and are in control throughout your appointment. When the mask is removed, the effects wear off within minutes.

It is one of the most widely used and most thoroughly studied sedation methods in dentistry, with an established safety record across patients of all ages including children and elderly patients.

How does nitrous oxide work?

Nitrous oxide works by slowing down your nervous system’s response to stress and discomfort. When you inhale a carefully calibrated mixture of nitrous oxide and oxygen through a small, comfortable mask placed over your nose, the gas reaches your brain quickly and begins producing a sense of calm and physical relaxation within two to three minutes.

The way nitrous oxide interacts with the brain is well understood. It influences certain receptor systems that regulate mood, anxiety, and pain perception, producing a sedative and mildly euphoric effect without causing unconsciousness. Your reflexes remain intact. Your ability to communicate with your dentist is unaffected. And because the gas is continuously mixed with oxygen throughout your appointment, the level of sedation can be adjusted in real time, increased slightly if you need more comfort, or reduced if you feel too relaxed.

What does laughing gas feel like?

Most patients describe the sensation as pleasant and noticeably different from their usual experience at the dentist. Common descriptions include a warm, tingling sensation in the hands, feet, or throughout the body, a feeling of heaviness or lightness in the limbs, a mild sense of floating or detachment, a general feeling of warmth and relaxation, reduced awareness of sounds or sensations in the mouth, and a sense of calm that makes it easier to stay still and comfortable throughout the procedure.

Some patients also experience mild euphoria, which is where the name laughing gas comes from. Not everyone laughs, but many people feel genuinely good during the appointment in a way that surprises them, particularly if they have spent years dreading dental visits.

Importantly, you remain aware of your surroundings throughout. You can hear your dentist, respond to questions, and signal if you need a pause. The sedation is intentionally mild. It takes the edge off anxiety and discomfort without taking away your alertness or control.

Who benefits most from nitrous oxide sedation?

Nitrous oxide is appropriate for a wide range of patients. At Scottsdale Dental Centre we recommend considering it in the following situations.

Patients with dental anxiety or dental phobia. If the anticipation of a dental appointment causes you significant stress, affects your sleep the night before, or has led you to cancel or avoid appointments in the past, nitrous oxide can make an enormous difference. Many patients who have avoided the dentist for years find that a single positive experience with nitrous oxide completely changes their relationship with dental care.

Patients with a sensitive gag reflex. A strong gag reflex can make even routine procedures uncomfortable and difficult to complete. Nitrous oxide reduces the gag reflex significantly, making impressions, x-rays, and clinical examinations much more manageable.

Patients undergoing longer or more complex procedures. For appointments involving multiple fillings, a crown preparation, wisdom tooth removal, or implant surgery, nitrous oxide helps patients remain comfortable and calm throughout a longer session. It also reduces muscle tension, which makes the procedure easier for both the patient and the dentist.

Patients who simply want a more comfortable experience. You do not need to have severe dental anxiety to benefit from laughing gas. Some patients choose nitrous oxide simply because they find dental appointments uncomfortable and would prefer a more relaxed experience. That is a completely valid reason and we are happy to discuss it with you.

Is nitrous oxide safe?

Yes. Nitrous oxide has one of the longest and most thoroughly studied safety records of any sedation method used in dentistry. It has been in clinical use for over a hundred and fifty years and is approved for dental use in Canada.

Several features of nitrous oxide make it particularly safe compared to other sedation options. It is administered continuously mixed with oxygen, so you are never breathing pure nitrous oxide. The level of sedation is adjustable in real time throughout the procedure. It does not affect your ability to breathe independently. It is not metabolized by the body and is simply exhaled, which means there is no liver or kidney burden. And it clears your system within minutes of the mask being removed, meaning you do not carry residual sedation effects for hours afterward as you would with oral sedation or IV sedation.

There are a small number of situations where nitrous oxide is used with additional caution or where alternative approaches may be preferred. Nitrous oxide vs oral sedation, what is the difference?

Scottsdale Dental Centre offers both nitrous oxide and moderate oral sedation. Understanding the difference helps you make an informed choice.

Nitrous oxide is inhaled through a nasal mask during your appointment. It takes effect within two to three minutes, can be adjusted in real time, and wears off within five to ten minutes of the mask being removed. You can drive yourself home afterward. It is appropriate for mild to moderate anxiety and for most routine to moderately complex procedures.

Oral sedation involves taking a prescribed medication before your appointment. It produces a deeper state of relaxation than nitrous oxide and is appropriate for patients with more significant anxiety or for longer, more complex procedures. The effects last several hours, meaning you will need someone to drive you home and you should plan to rest for the remainder of the day.

Dr. Ahmad will discuss both options with you and recommend the approach most appropriate for your specific needs, your level of anxiety, and the procedure involved. For many patients, nitrous oxide is entirely sufficient. For patients with more significant anxiety, oral sedation may be the better choice. In some cases, a combination of both is used.

How to prepare for a nitrous oxide appointment

Preparation for a nitrous oxide appointment is straightforward. Avoid eating a heavy meal in the two hours before your appointment. Light food and clear fluids are fine. You simply want to avoid arriving on a very full stomach, which can occasionally contribute to mild nausea in a small number of patients. Continue taking any doctor-prescribed medications as normal unless your dentist has specifically advised otherwise.

Make sure your nasal passages are reasonably clear on the day of your appointment, as nitrous oxide is administered through a nasal mask. If you have a heavy cold or significant nasal congestion, let us know when you call and we can discuss whether to proceed or reschedule.

Wear comfortable, loose clothing. Arrive a few minutes early if it is your first time using nitrous oxide so there is no rush. Tell your dentist or dental assistant if you have had nitrous oxide before and how you responded to it. This helps us calibrate appropriately from the start.

What to expect after your appointment

One of the most practical advantages of nitrous oxide sedation is the recovery time. Because the gas leaves your system within minutes of the mask being removed, you will feel essentially normal very quickly after your appointment. Most patients are ready to leave the practice and resume their day within ten to fifteen minutes.

Unlike oral sedation, you can drive yourself home after a nitrous oxide appointment. You can return to work, pick up children from school, or continue with your normal afternoon and evening. There are no dietary restrictions after the appointment and no need to arrange for someone to be with you.

A small number of patients experience mild nausea or lightheadedness immediately after a nitrous oxide appointment. This typically resolves within a few minutes and is usually related to the transition off the gas or, occasionally, to having eaten a very heavy meal beforehand.
